Human Connections is a Medicaid provider agency licensed through the Oregon Department of Developmental Disability Services (ODDS). Our primary focus is working with individuals who receive case management services through the brokerage or community developmental disability programs. With a team of dedicated, experienced and compassionate professionals, we work directly with the individual and their identified team to work towards goals and increase autonomy and independence in the lives of every person we are lucky to assist.

Human Connections is owned and operated by behavior professionals which provides the unique opportunity for each employee and client to have access to the education and resources needed to provide proactive supports and work through challenges. Operating with the understanding that a person is not the sum of their challenges, we seek to positively understand behaviors as a means of bridging gaps and allowing people to actively work towards their identified goals.

With a core belief in empowering each person we work with, Human Connection consistently strives to provide individualized services which work towards goal achievement, personal growth and the establishment of meaningful connections. Any person can achieve success when connected with people and resources who focus on strengths, while also anticipating and working through challenges, in a realistic way.

Human Connections was established in 2017 to address a need for services which were able to anticipate and work through challenging behaviors. Individuals seeking services who have a history of engaging in challenging behaviors are often turned away from programs due to underlying behavioral barriers.
